MPA PREPARATION & UNDERWRITING CHECKLIST

Missing documents are the #1 reason deals get stuck in underwriting. Do not submit the MPA until every item on this list is secured.

[ ] 1. Merchant Processing Agreement (MPA)
    - All fields completed.
    - Legal entity name matches state registry EXACTLY.
    - Wet or digital signature from a guarantor with >25% equity.

[ ] 2. Voided Check / Bank Letter
    - Must be pre-printed. NO temporary checks.
    - If using a bank letter, must be on official letterhead, signed by branch manager.
    - Name on check must match the Legal or DBA name on the MPA.

[ ] 3. Identity Verification
    - Clear, color scan of the Principal's Driver's License or Passport.

[ ] 4. Business Verification
    - Articles of Incorporation OR State/Local Business License.
